Reimbursement for Conference Funding Submission Form
This form is to be used AFTER you have been approved for conference funding AND attended your conference. All sections must be answered in order to receive your reimbursement. Please note that once this form is received, it will be reviewed by multiple parties and your submissions will be available for use for outlets such as the the LKSOM Newsletter, Temple Alumni Association, Student Government Association, Student Affairs, and other branched of LKSOM. YOU MUST PROOFREAD ALL PORTIONS OF THIS FORM BEFORE SUBMISSION. Failure to submit this form will result in loss of funding for your conference. CONFERENCE REFLECTION REQUIREMENTS: This reflection is a new pilot program to allow students to share their research/experiences with the student body.
How would you explain your research topic to a patient? What were some of the topics that you learned about at the conference? How has this experience of presenting or attending a conference allowed for your growth as a future physician? How can this experience benefit the student body of LKSOM?

- Must be proofread for spelling and grammar errors
- All submissions will be subject to LKSOM Honor Code
